Spherocylindrical and double spherical neutralization retinoscopy are techniques used in the process of subjective refraction to determine the best corrective lenses for an individual's refractive error. These techniques help in prescribing accurate spectacle or contact lens prescriptions by neutralizing the patient's refractive error.

 

Sphero-cylindrical Neutralization Retinoscopy:

Spherocylindrical neutralization retinoscopy is performed when the patient has astigmatism, which is a refractive error characterized by an irregular curvature of the cornea or lens. The purpose of this technique is to determine the power and axis of the cylindrical lens needed to correct the astigmatism.

 

During spherocylindrical neutralization, the retinoscopist uses a streak retinoscope and a trial frame. The retinoscopist starts by placing a spherical lens in front of the patient's eye to bring the retinal reflex into focus. The retinoscopist then adds a cylindrical lens and rotates it until the orientation of the streak of light is aligned with the axis of the astigmatism. This process helps neutralize the astigmatic error, and the combination of the spherical and cylindrical lenses determines the patient's prescription.

 

Double Spherical Neutralization Retinoscopy:

Double spherical neutralization retinoscopy is used when the patient does not have significant astigmatism, and their refractive error is primarily spherical in nature. The objective is to determine the power of the spherical lens needed to correct the refractive error.

 

In double spherical neutralization, the retinoscopist also uses a streak retinoscope and a trial frame. Starting without any lenses, the retinoscopist moves closer or farther from the patient while observing the movement of the retinal reflex. By adding plus or minus spherical lenses, the retinoscopist brings the retinal reflex to neutral or zero movement, indicating that the patient's refractive error has been neutralized. The lens power at this point represents the spherical component of the patient's prescription.
