Divergence insufficiency

 Divergence insufficiency is a specific type of binocular vision disorder characterized by an inability to maintain proper eye alignment and coordination when looking at objects in the distance. Here are the key aspects of divergence insufficiency:

 Definition:

Divergence insufficiency refers to a condition where the eyes have difficulty diverging or moving outward to maintain binocular vision during distance viewing. It is characterized by an exodeviation (outward deviation) of the eyes that is greater at distance compared to near fixation.

 Etiology:

The exact cause of divergence insufficiency is not fully understood. However, it is believed to be related to age-related changes in the extraocular muscles, particularly the muscles responsible for eye divergence. Neurological factors, such as cranial nerve palsies or brainstem lesions, may also contribute to the development of divergence insufficiency in some cases.


 Clinical Features:

Exodeviation: Divergence insufficiency is characterized by an exodeviation (outward deviation) of the eyes that is more pronounced at distance fixation compared to near fixation. The exodeviation typically increases when the patient looks far into the distance.

Diplopia (Double Vision): Patients with divergence insufficiency may experience diplopia or double vision, particularly when looking at distant objects.

Asthenopia: Eye strain, fatigue, and discomfort may occur, especially during prolonged distance viewing.

Normal Near Vision: Near vision is typically maintained without significant deviation or symptoms.

 

Diagnostic Tests:

Cover Test: A cover test is performed to assess the alignment of the eyes during distance fixation. It helps identify the exodeviation and any deviation changes when comparing distance and near fixation.

Prism Bar Test: Prism bar testing can determine the amount of exodeviation and assess the near-to-distance disparity in eye alignment.

Binocular Vision Assessment: A comprehensive evaluation of binocular vision function is important to rule out other related conditions and assess the overall binocular visual system.

 

Treatment:

The treatment of divergence insufficiency aims to alleviate symptoms and improve binocular vision. It may include:

 

Prism Glasses: Prescribing prism glasses can help align the eyes and reduce diplopia and asthenopia.

Vision Therapy: Eye exercises and vision therapy techniques can be employed to improve eye coordination and strengthen the extraocular muscles.

Base-Out Prism: Temporary use of base-out prism glasses can alleviate symptoms in some cases.

Management of Underlying Conditions: If an underlying neurological condition or cranial nerve palsy is contributing to divergence insufficiency, appropriate management of the underlying cause may be necessary.
